 DEED OF AGREEMENT

 

This DEED OF AGREEMENT is intending to provide Ambulance & Medical Service on emergency, made on ----------------------------------------------------------.

 

BETWEEN

 

TM Designers Ltd. Shilmun, Nimtali, Tongi, Gazipur, Bangladesh; represented by its Managing Director Mr. Tawfique (hereinafter be called as the Factory owner) is  the First Party.

 

AND

 

-------------- (Pvt.) Ltd, Gazipur, Bangladesh; represented by its Managing Director Dr. ------------ (hereinafter be called Service provider) is the Second Party.

 

First party proposed to provide medical service with Ambulance Services on emergency to the Second party under following terms and conditions:

 

TERMS & CONDITIONS

 

1.    That in pursuance of the aforesaid offer and acceptance, the first party will receive Ambulance and Medical Service and second party will provide during emergency on call.

 

2.    Described in the schedule proposed to let out the same for a period of five years facility has agreed to the said proposal and accept the offer of the first party.

 

3.    The second party will receive and attend all accidental and emergency patients from reference of the first party.

 

The agreement is made in two exact copies one is kept with the First party the other with the Second party. Both the First party and the Second party hereto have well understood its contents.
